WHAT IS CONCIERGE MEDICINE?
WHY SHOULD I CHOOSE CONCIERGE MEDICINE?
HOW ARE YOU DIFFERENT FROM MY CURRENT DOCTOR?
DO YOU ACCEPT INSURANCE?
We do work with insurance, but differently. Our patients pay a separate monthly membership fee which gives them unlimited access to all of our services and concierge benefits. You do not need insurance to be a patient of our practice. There are no additional charges to the member for our physician services. Typically, membership fees are not covered by restrictive health insurance plans.
HOW MUCH DOES CONCIERGE MEDICINE MEMBERSHIP COST?

HOW DO I PAY FOR THE MEMBERSHIP FEE?
We offer flexible payment options for your convenience. We accept all major credit cards, automated bank draft, and you can use an HSA account as well.
WHAT DOES THE MEMBERSHIP FEE COVER?
- 24/7 access to Dr. Nussbaum via call, text, and video chat- Same day and next day appointments
- Timely, unrushed appointments—we schedule our appointments in 1-hour blocks
- All of your lab work can be done in the office.
- Home visitation available by request
- Assistance with specialists: we arrange all of your outside medical visits for you—all you have to do is show up
- Hospital/inpatient care: Dr. Nussbaum is a Fellow of Hospital Medicine which means when you are hospitalized, your primary care doctor is there every step of the way to ensure your needs are met
- Holistic, preventative care
- Remote patient monitoring: we can monitor your pulse, blood pressure, oxygen saturation and do a 6 lead EKG through a device we give you
WHAT IS NOT COVERED BY THE MEMBERSHIP FEE?
Your health insurance plan typically covers most costs of diagnostics, medications, hospitalizations, specialist visits, and immunizations. If you are not insured, we will work with you to find affordable cash pay options for those items not covered by the membership fee.
WILL MY INSURANCE COVER THE MEMBERSHIP FEE?
We do not bill your private insurance for the membership fee. Members pay this directly to Insight Life Care.
DO I HAVE TO PAY A COPAY EVERY TIME I SEE THE DOCTOR TOO?
No, there are no additional fees beyond your monthly membership fee.
DO I HAVE TO SIGN A CONTRACT?
Our initial trial period is 90 days, after which members may continue on a month-to-month basis.
IS THERE A MEMBERSHIP OPTION FOR CHILDREN?
Insight Life Care is an adult care practice focusing on individuals age 18 years and older.
CAN I MEET DR. NUSSBAUM BEFORE JOINING?

WHAT HAPPENS WHEN DR. NUSSBAUM ISN'T AVAILABLE?
When Dr. Nussbaum is unavailable for any reason, trusted physician colleagues are on call for our patients. Uniquely, our practice includes a licensed and experienced Emergency Room nurse to assist in every aspect of your care.
IS THERE A LIMIT ON VISITS IN A GIVEN MONTH OR YEAR?
You may see the doctor as often as medically necessary as part of your membership.
WHAT IF I HAVE A MEDICAL QUESTION WHILE I AM TRAVELING OUT OF STATE/COUNTRY?
We have a robust telemedicine platform which allows us to schedule appointments with patients wherever they are! Just let us know that you are traveling, and we will schedule a virtual appointment for you.
WHAT IF I NEED TO SEE A SPECIALIST?
We will assist you in scheduling, managing, and following up with any specialist needed. Whether you have health insurance or not, we arrange and expedite consultations when necessary within your health insurance plan or self-pay budget.
WHAT HAPPENS IF I'M HOSPITALIZED?
Dr. Nussbaum is a Fellow of Hospital Medicine and on staff at two of our local hospitals, Brandon Regional and South Bay. He admits and follows all his patients at these facilities daily. If you are admitted elsewhere, he will be in close contact with your care team and ensure all of your needs are met. Dr. Nussbaum stays involved in every facet of your care and will communicate with you regularly.
CAN I REQUEST A HOUSE CALL?

Yes, we are happy to care for all patients wherever they may work or dwell.
